Jose M. Diaz, age 35, died after slipping and falling from a narrow ledge while cleaning. He was climbing a small ladder onto a ledge when he either tripped or slipped on a steel rail. He fell 120 feet and died instantly from head and neck injuries.

Diaz was employed by Superior Environmental Solutions. OSHA is investigating. The steel mill in Lorain has been cited 19 times in the last five years for serious violations.

Diaz was originally from Puerto Rico. He has six-year-old twin girls back in Puerto Rico that he had been providing for.
